Too often, I see patients who have been given a simplistic, one body part diagnosis that was arrived at solely on the basis of imaging results and a brief discussion with a health care provider. By the time these patients get to me, many of them have been treated unsuccessfully by the likes of injections, medications, surgery, or even physical therapy! Why is this the case? In my experience, as the medical field has become more and more specialized, evaluating the body as a whole has become a lost art (a shoulder doctor will likely find something wrong with your shoulder!). In addition, our health care system is so overburdened and underfunded that many Providers simply do not have the time to do a thorough examination. Thus, they become over-reliant on medical imaging as the sole source of diagnosis. That's not to say the modern medical imaging isn't a fantastic diagnostic tool, it lets us see the intneral workings of our body in a way we would otherise not be able to, but it cannot (and was never intended to be) the sole means of diagnosing pain disorders. We now have many large studies over decades that have shown that there is very poor correlation between imaging results and a persons level of pain and disfunction (not to mention that there is little consistency between radiologists in interpretic orthopaedic imaging!). What imaging does not show us is how a person moves and postures, which muscles may be weak or tight, and the effect of dysfunction of seemingly unrelated body parts on the rest of our body. These issues can only be fully appreciated with a proper clinical exam in which movement quality, posture, and neuromuscular control are all assessed thoroughly. Once a proper movement diagnosis has been acheived, we can then use manual therapy, nueromuscular facilitation, and movement re-education technqiues to optimize function.
